Who may qualify?
You may qualify if you used Flo in the U.S. from Nov. 1, 2016 to Feb. 28, 2019 and entered period or pregnancy data.

How much could I get?
Share from a $59.5 million fund, California residents get double the standard share. Payment is not known yet. It depends on valid claims, fees, costs, court approval, and whether you are in the California Subclass.

The form asks for contact info, a Unique ID and PIN if you have one, and a sworn statement about using Flo.

Where to look for your claim ID or notice
Look for an email or mail notice with “Unique ID” and “PIN” near the top. The sample shows “Unique ID: XXXXX” and “PIN: XXXXXXXXX.”
