Eligibility
Who may qualify?
You may qualify if you indirectly paid for a listed generic drug from May 1, 2009 through December 31, 2019, outside Indiana or Ohio.

You must list each covered drug, estimate your total out-of-pocket spending, and provide proof for at least one purchase of each drug.
